Adult Degenerative Lumbar Scoliosis — Correction Step by Step
🔎 Start with the problem.
A 61-year-old patient with adult degenerative lumbar scoliosis. The goal is to restore function, balance, and decompression of neural elements.
🧠 First, treat the neurology.
Address the symptomatic stenotic levels with adequate decompression and interbody reconstruction. Neural decompression comes before deformity correction.
📐 Then rebuild the sagittal profile.
Restore segmental lordosis where it matters most — particularly at L4–S1. In this case, distal lordosis improved from 25° to 40° with a PI of 50°.
➡️ Sagittal first, coronal second.
Once the distal lumbar foundation is restored, complete the sagittal correction before focusing on the coronal deformity. Alignment is a sequence, not a single maneuver.
↔️ Now correct the coronal plane.
Use asymmetric forces deliberately: distraction on the concavity and compression on the convexity to translate and derotate the lumbar curve while maintaining the sagittal correction already achieved.
⚖️ Never sacrifice one plane to correct another.
Coronal correction should complement — not destroy — the reconstructed lumbar lordosis. The objective is a balanced three-dimensional spine, not merely a straight AP X-ray.
✅ Final X-ray. Final check. Then leave the OR.
Before leaving the operating room, verify coronal balance, sagittal alignment, pelvic relationship, distal lordosis, and the overall construct. The operation is finished only when the planned alignment is actually achieved.
🧠 Treat the neurology → 📐 Restore L4–S1 lordosis → ➡️ Correct sagittal alignment → ↔️ Correct coronal deformity → ✅ Confirm global balance.
Adult spinal deformity surgery is about restoring balance — in the right order. 👊🏻

Her ortopedi kökenli omurga cerrahının en eski işi travmadır👊🏻
Complex Humerus Fractures: Respect the Biology
✅Wide deltopectoral exposure may be necessary. In severely comminuted humeral fractures, adequate visualization is essential for safe and accurate reconstruction.
✅The radial nerve must be identified and protected. With extensive exposure and complex fixation, careful exploration of the radial nerve is a critical step.
✅Don’t strip the fragments. Every fragment carries its own blood supply through the surrounding soft tissues.
✅Preserve these attachments whenever possible.
Think LEGO — or a 3D puzzle. Complex fractures are reconstructed patiently, fragment by fragment, restoring length, alignment and rotation without sacrificing biology.
✅Sometimes two plates are not enough. Certain highly comminuted fractures may require three-plate fixation to achieve sufficient stability. The number of plates matters less than achieving the right mechanical environment.
✅Respect biology, provide stability, and let the bone heal. Successful fracture surgery is not simply about putting metal on bone. It is about finding the balance between biology and biomechanics.
✅Complex fracture. Careful reconstruction. Stable fixation. Preserved biology. Solid union.

Adult spinal deformity surgery is not only about straightening the spine. It is about restoring balance.
🔹 Understand the deformity: Adult lumbar scoliosis is a complex 3D problem, involving both coronal and sagittal imbalance.
🔹 Plan the correction strategically: Concave-side interbody cage placement combined with convex-side compression can provide powerful segmental correction.
🔹 Level the foundation: Restoring the orientation of the lower lumbar vertebrae—particularly L4—is critical for rebuilding a balanced lumbar spine.
🔹 Respect sagittal alignment: More lordosis is not always better. Overcorrection can be as problematic as undercorrection. The goal is patient-specific alignment.
🔹 Use circumferential reconstruction when necessary: Anterior/lateral interbody support combined with posterior fixation can restore both coronal and sagittal balance.
🔹 The ultimate goal is not a perfect X-ray. It is a balanced, stable and functional spine with durable correction.
Correct the deformity. Restore the balance. Respect the biomechanics.
Dengeyi yeniden kur. Biyomekaniğe saygı göster

Not Every Kyphosis Should Be Corrected the Same Way
Successful kyphosis surgery is not about achieving the maximum correction—it is about achieving the safest correction for the individual spinal cord.
🧠 1. Continuous Intraoperative Neuromonitoring
Motor Evoked Potentials (MEPs) and Somatosensory Evoked Potentials (SSEPs) are monitored throughout the procedure. Even subtle signal changes can indicate spinal cord stress and immediately influence our surgical strategy.
📐 2. The Amount of Correction Must Be Individualized
The goal is not to create the straightest spine possible, but to restore optimal sagittal alignment while respecting the neurological limits of each patient.
🧬 3. Spinal Cord Morphology Matters
Patients with Lenke Type 3 spinal cord morphology often have a more vulnerable spinal cord at the apex of the kyphosis. In these cases, the correction strategy should be tailored to the spinal cord rather than the deformity alone.
⚠️ 4. Neuromonitoring Guides Every Critical Decision
If neuromonitoring signals deteriorate, correction may be reduced, instrumentation adjusted, spinal cord perfusion optimized, or additional decompression performed. Patient safety always takes priority over radiographic perfection.
🏆 5. A Successful Operation Means More Than a Beautiful X-ray
The true success of kyphosis surgery is a balanced spine, preserved neurological function, and a patient who wakes up safely with improved posture and quality of life.
“The best correction is not the maximum correction—it is the safest correction for that patient’s spinal cord.”
